CU CHAMPION 2017
Background
CU CHAMPION 2017 refers to the community outreach events that scheduled within October 2016 to September 2017. CU CHAMPION volunteers will transform their passion into action as the team's slogan , Passion in Action, stipulated the team's core value.
Since October 2016, CU CHAMPION has been conducting a series of Post Outreach Follow Up Visits for 200 elderly subjects. The visits involve Core Team, Clerkship Task Force members and registered pharmacists. The group members provide individual disease and medication consultation for these subjects twice in six months. All follow up visits shall be completed by the end of March 2017.
CU CHAMPION will be involved in more health check and health education events in 2017. The services and target subjects include but not limited to: health and medication safety education events for primary and secondary school students, ethnic minority, and elderly subjects.
